Overview

Knee replacement, also known as Total Knee Arthroplasty (TKA), is a surgical procedure whereby the diseased knee joint is replaced with artificial material. The knee is a hinge joint that provides motion at the point where the thigh meets the lower leg. The thighbone (or femur) abuts the large bone of the lower leg (tibia) at the knee joint. During a total knee replacement, the end of the femur bone is removed and replaced with a metal shell. The end of the lower leg bone (tibia) is also removed and replaced with a channelled plastic piece with a metal stem. Depending on the condition of the kneecap portion of the knee joint, a plastic "button" may also be added under the kneecap surface. The artificial components of a total knee replacement are referred to as the prosthesis. The posterior cruciate ligament is a tissue that normally stabilizes each side of the knee joint so that the lower leg cannot slide backwards in relation to the thighbone. In total knee replacement surgery, this ligament is either retained, sacrificed, or substituted by a polyethylene post. Each of these various designs of total knee replacement has its own particular benefits and risks.

When a knee replacement is needed

Knee replacement surgery is usually necessary when the knee joint is worn or damaged to the extent that your mobility is reduced and you experience pain even while resting. The most common reason for knee replacement surgery is osteoarthritis. Other conditions that cause knee damage include:
  • rheumatoid arthritis
  • haemophilia
  • gout
  • disorders that cause unusual bone growth (bone dysplasias)
  • death of bone in the knee joint following blood supply problems (avascular necrosis)
  • knee injury
  • knee deformity with pain and loss of cartilage

Causes of Knee Pain

The most common cause for knee pain and limitation in mobility is arthritis. There are 3 kinds of arthritis:

  • Osteoarthritis is the commonest and age-related disease-causing wear and tear of the knee joint. It usually occurs over 50 years of age, where the cartilage cushioning the joint bones can wear away and bones soften causing easier and faster attrition of bone with symptoms like pain, stiffness, and swelling.
  • Rheumatoid arthritis is autoimmune inflammatory arthritis where the synovial membrane surrounding the joints gets inflamed and thick. This, in turn, damages the cartilage and the underlying bone causing pain, stiffness, swelling, and deformity.
  • Post-traumatic arthritis is caused due to the serious knee injury. Fractures and ligament tears can damage the articular cartilage causing knee pain and limited motion.

Treatment options

Treatment alternatives to total knee replacement include:

  1. Nonsurgical treatment for osteoarthritis/arthritis includes:
    • Physical therapy
    • Analgesics and anti-inflammatory medications and suitable drugs in cases of infections of the joints
    • Knee bracing or shoe inserts which help in knee alignment and balancing of weight on the knee joints.
    • Visco-supplementation injections for lubricating the joint to make the joint movement less painful.
  2. Surgical alternatives include:
    • Arthroscopy is a minimally invasive procedure where small incisions can be done to repair the damage with arthroscopy.
    • Osteotomy is a surgical procedure where the involved leg bone is cut, realigned, and allowed to heal. It is useful for patients older than 60 with inflammatory arthritis.
    • Partial knee replacement involves replacing only the part of the knee joint that is damaged or arthritis.
